I certify that all information provided in this credit application is true and complete. I authorize GDT Holdings LLC d/b/a Delta Tire (“Delta Tire”) to provide this application to Shield RTO Holdings LP d/b/a Shield Leasing (“Shield”) (an affiliate of Delta Tire), and Delta Tire and Shield may rely on this information to make inquiries for the purpose of evaluating and verifying this credit application, including obtaining consumer reports from any consumer reporting agency and contacting my employer. If Shield is unable to approve my application for credit, I authorize Delta Tire, on my behalf, to forward my credit application, and the results of my application to nonaffiliated third parties that may assist me in obtaining rental-purchase financing, which nonaffiliated third parties may also obtain a consumer report for this purpose. I acknowledge that the information I have provided in this credit application constitutes my credit inquiry or application to Delta Tire, any such nonaffiliated third party, and to any arranger of credit or creditor to which my information may be disclosed to process my inquiry or application. I further expressly agree that any arranger of credit or creditor who receives my information may contact me at the calling numbers I have provided, including my cellular phone number, using a live caller, an automated dialer, or an artificial or prerecorded voice. I intend this consent to be effective for calls other than sales or solicitation calls, even if my calling numbers are included in a do-not-call list or registry. Finally, if Shield or another creditor grants my request for credit and I sign a rental-purchase agreement, I also authorize and give consent to the creditor who enters into a rental-purchase agreement with me to obtain my consumer report from any consumer reporting agency at any time during the life of my rental-purchase agreement for any purpose, including but not limited to times when I ask such creditor to defer my payment(s) or extend my payment due date(s) under my rental-purchase agreement, in connection with any action related to collection of any amounts owing under my rental-purchase agreement, or for other legitimate purposes associated with my rental-purchase agreement for which there is a permissible purpose under the federal Fair Credit Reporting Act.

CALIFORNIA RESIDENTS: If married, you may apply for credit separately as an individual.

MAINE RESIDENTS: You have free choice in the selection of the agent and insurer through or by which the insurance in connection with the requested rental-purchase agreement is to be placed. Your decision to obtain any insurance product from an agent, broker or insurer of your choice will not affect the credit decision unless the insurance product you choose does not comply with the creditor’s reasonable requirements.

NEW HAMPSHIRE RESIDENTS: If you are applying for a balloon payment contract, upon request and before entering into the balloon payment contract, you are entitled to receive a written estimate of the monthly payment amount for refinancing the balloon payment in accord with the creditor’s existing refinance programs. A balloon contract is an installment sale contract with a scheduled final payment that is at least twice the amount of one of the earlier scheduled equal periodic installment payments.

NEW YORK RESIDENTS: In connection with this application, we may request a consumer report on you. If you request, we will inform you whether or not a consumer report was requested and, if it was, of the name and address of the consumer reporting agency that furnished the report. Additional consumer reports may be ordered without further notice to you in connection with any update, renewal or extension of credit granted.

OHIO RESIDENTS: The Ohio laws against discrimination require that all creditors make credit equally available to all creditworthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio civil rights commission administers compliance with this law.

RHODE ISLAND RESIDENTS: Credit Reports may be obtained in connection with this application for credit.

VERMONT RESIDENTS: You authorize Delta Tire and any creditor with whom this application is shared, and each of their respective employees or agents, to obtain and verify information about you (including one or more credit reports, information about your employment and banking and credit relationships) that they may deem necessary or appropriate in evaluating your credit application. If your application is approved and credit is granted, you also authorize the parties granting credit and/or holding your account, and their respective employees and agents, to obtain additional credit reports and other information about you in connection with reviewing the account, increasing the available credit on the account (if applicable), taking collection on the account, or for any other legitimate purpose associated with the account.

WISCONSIN RESIDENTS: No provision of a marital property agreement, a unilateral statement under Wis. Stat. 766.59, or a court decree under Wis. Stat. 766.70 applied to marital property adversely affects our interest unless you furnish a copy of such agreement, statement, or decree to us or we have actual knowledge of such adverse provision before credit is granted.
